The RN, Patient Access is responsible for the clinical coordination
of assigned hospice evaluations and/or admission of eligible
patients in all care settings including establishing the initial
Plan of Care and urgent symptom management. The RN is responsible
for case management and daily care coordination for any
hospitalized patient.
In addition to RN, Patient Access responsibilities, the RN, Patient
Access Preceptor plans, coordinates and carries out educational
opportunities for Company team nurses. S/he orients the team nurses
to their role within the interdisciplinary team; evaluates job
performance using competency checklist to insure each new nurse has
successfully acquired knowledge and skills essential to providing
patient care.
Qualifications: --- Bachelor's degree in nursing or related field
or an equivalent combination of education and experience ---
Current Florida license as RN, current active single-state
out-of-state nurse license, or current active multistate/compact
nurse license. --- Minimum of two (2) years' RN experience ---
Minimum of one (1) year experience in ER, ICU or similar setting
requiring advanced assessment skills, multitasking, critical
decision making and problem solving skills --- One (1) year of
Hospice or home health experience preferred --- Previous experience
working with an EMR/EHR (Electronic Medical/Health Record) system
--- Mobile Driver - Valid driver's license and automobile insurance
per Company policy --- Reliable transportation to meet schedule ---
Ability to effectively communicate in English (written and verbal)
--- Flexible work schedule including the ability to work weekends
--- Ability to physically access any care setting (i.e., navigate
stairs and narrow spaces, tolerate heat and lack of air
conditioning) in any area Company serves --- Ability to manage the
emotional stress of working with and caring for terminally ill
patients and their families --- Ability to perform the essential
functions and physical requirements (including, but not limited to:
lifting patients and/or equipment, bending, pushing/pulling,
kneeling) of the job with or without reasonable accommodation

Responsibilities: --- Evaluates every hospice referral on same day
as referral, unless otherwise indicated, utilizing approved
criteria and guidelines. --- For hospice eligible patients,
completes Initial Assessment, initiate initial Plan of Care and
handle urgent symptom management; ensures all documentation is
completed and signed with accuracy, timeliness, legibility,
professional terminology and awareness of legal and regulatory
implications within 2 hours of visit end time. --- Demonstrates
clinical skills regarding physical assessment, symptom management,
patient/family relationships, and case management; consults,
refers, and collaborates with other professionals to promote
quality and continuity of patient/family care. --- Provides
information regarding available community resources and other
community resources when hospice care is not appropriate. ---
Provides timely and consistent feedback to referral sources,
physicians, and patient/families regarding care plan. --- May
provide home/facility visits in all clinical settings, if no
admission visits are scheduled during the regular work day. ---
Provides complete report to the team who will assume care after the
admission is completed. --- Cultivates, develops and enhances care
relationships both internally and externally. --- Completes
education requirements timely including Annual Skills and Annual
Education. --- Performs other duties as assigned.
